Hubs for Continuity of Care

While our Mobile Medical Units (MMUs) are designed for initial outreach and primary diagnosis, we recognize the importance of fixed, local facilities for ensuring the continuity of care. Kavach works to strengthen and integrate with existing public and private Primary Healthcare Centres (PHCs). These centres serve as crucial local hubs for follow-up consultations, ongoing management of chronic conditions, and a stable point of contact for the communities we serve.

Our Role in the PHC Ecosystem

Patient Referrals

Patients identified by our MMUs requiring non-urgent, ongoing care are referred to the nearest partner PHC, ensuring their health journey continues seamlessly.

Data Integration

A patient's Kavach Digital Health Passport can be securely accessed by authorized personnel at partner PHCs, providing a complete medical history and avoiding redundant tests or diagnoses.

Capacity Building

We aim to partner with local health authorities to support PHCs with technology, training, and streamlined referral pathways, strengthening the overall public health infrastructure.
